No Referal Needed (Minnesota Direct Access):
In the state of Minnesota, you have "Direct Access" to physical therapy. This means you can be evaluated and treated by a licensed Physical Therapist without a formal referral or prescription from a physician. We will coordinate with your healthcare team should your condition require medical intervention or if medical treatment extends beyond the 90-day direct access period.
No Insurance accepted:
We Work for You, Not Your Insurance Company! At Solid Health, we believe your care should be decided by you and your medical team—not the insurance company. By not participating in insurance networks, we are able to provide 60-80 minutes of one-on-one care with a Doctor of Physical Therapy without the headache. This allows us to focus entirely on your goals, resulting in more personal experience.
Good Faith Estimate:
Under the law, healthcare providers must give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a ‘Good Faith Estimate’ for the total expected cost of any non-emergency items or services. All our pricing is up front and the only added tax is what is required by the State of Minnesota.
MinnesotaCare Provider Tax Disclosure:
In accordance with Minnesota law, a 1.8% MinnesotaCare Provider Tax is applied to all health care services provided. This tax supports the state's health care programs and is included in our transparent, upfront pricing.
Medicare Disclosure:
Solid Health LLC is not a Medicare provider, however we are able to perform wellness, fitness, and prevention services to all Medicare and non-Medicare patients. Medicare prohibits cash payments for services that would normally be covered under Medicare. Please contact us if you have question about this.
HIPAA & Privacy Statement:
Your privacy is our priority. Any information shared with us prior or during your treatment is protected in accordance with professional ethical standards and HIPAA guidelines. We use secure, encrypted platforms for scheduling and documentation to ensure your personal health information remains confidential.
Licensure:
Solid Health LLC operates as an independent physical therapy practice. Tim and Laura Hafemeyer are licensed Physical Therapists in the State of Minnesota. We operate as independent practitioners.
Minnesota Home Care Bill of Rights:
As providers of home physical therapy in the state of Minnesota, we are committed to upholding the Minnesota Home Care Bill of Rights (MN Statute 144A.44).
